Three Midlife Health Factors May Delay Dementia by More Than a Decade, Study Finds

Research suggests controlling blood pressure, preventing type 2 diabetes, and avoiding smoking during middle age could add nearly 13 years of life without dementia. The choices people make in their 40s, 50s, and early 60s may have a greater impact on brain health than previously understood.

A large, long-term study published in Neurology found that adults who reached midlife without three common cardiovascular risk factors including high blood pressure, type 2 diabetes, and smoking lived nearly 13 more years free of dementia than those with all three risk factors. The findings add to growing evidence that protecting cardiovascular health may also help preserve cognitive function later in life.

Researchers say the results reinforce the importance of identifying and managing vascular risk factors well before memory problems emerge. While no single intervention can prevent dementia, addressing modifiable health risks during middle age could significantly delay its onset for many people.

A Long-Term Look at Brain Health

The study analyzed data from 12,409 participants enrolled in the Atherosclerosis Risk in Communities (ARIC) Study, an ongoing U.S. research project that has followed adults for decades to better understand cardiovascular disease and healthy aging.

Participants entered the analysis at an average age of 56 and had no evidence of dementia at enrollment. Investigators evaluated whether each participant had three common vascular risk factors:

  • High blood pressure
  • Type 2 diabetes
  • Current smoking

Participants were then followed for approximately 26 years. During that time, more than 3,000 people developed dementia, while more than 5,000 died without receiving a dementia diagnosis. Rather than simply estimating the likelihood of developing dementia, researchers examined how many years participants were expected to live free of the condition based on their cardiovascular risk profile during middle age.

Fewer Risk Factors, More Years Without Dementia

The findings showed a clear relationship between cardiovascular health and dementia-free survival.

Adults with no vascular risk factors averaged approximately 30 years of life without dementia after midlife. Those with one risk factor averaged about 26 years, participants with two risk factors averaged 21 years, and individuals with all three averaged just over 17 years.

Taken together, adults with healthy blood pressure, no diabetes, and no history of smoking experienced nearly 13 additional years free from dementia compared with those who had all three risk factors.

Although the study was observational and cannot prove that these factors directly cause dementia, the consistent relationship across thousands of participants strengthens existing evidence that vascular health plays a critical role in healthy cognitive aging.

Senior investigator Dr. Josef Coresh, director of the Optimal Aging Institute at NYU Langone, said the findings support addressing these modifiable risk factors long before older adulthood.

“Our findings argue that people need to actively avert these factors in midlife as a strategy for preserving brain health for more than a decade,” he said, adding that he hopes the results encourage people to stop smoking and closely monitor their vascular health beginning around age 45.

Why Researchers Focused on Midlife

Much dementia research has focused on adults after age 65, when symptoms often begin to appear. This study shifts attention to an earlier period of life, when prevention efforts may have the greatest impact.

Changes associated with Alzheimer’s disease and other forms of dementia can begin years before noticeable memory problems develop. By targeting modifiable risk factors between the ages of 45 and 65, clinicians may have an opportunity to influence long-term brain health before irreversible damage occurs.

The researchers noted that hypertension, diabetes, and smoking commonly emerge during middle age and can contribute to chronic injury of the brain’s blood vessels over time.

The Heart-Brain Connection

The study reinforces an increasingly recognized concept in medicine: what benefits the cardiovascular system often benefits the brain.

Healthy blood vessels deliver oxygen and nutrients essential for normal brain function. When vascular health declines, the brain may become more vulnerable to chronic inflammation, reduced blood flow, small vessel disease, and stroke, all of which have been associated with cognitive decline.

Each of the three risk factors evaluated in the study affects vascular health differently.

Persistent high blood pressure can damage the delicate blood vessels that supply the brain, while diabetes contributes to inflammation, vascular injury, and metabolic dysfunction. Smoking accelerates atherosclerosis, reduces oxygen delivery, and increases the risk of both cardiovascular disease and stroke. Although each condition independently increases health risks, they frequently occur together, amplifying their cumulative effect on long-term outcomes.

Differences Among Population Groups

Researchers also observed differences among demographic groups.

Women generally lived more years free of dementia than men, regardless of how many vascular risk factors they had. White participants also experienced more dementia-free years than Black participants.

The investigators cautioned that these findings likely reflect the influence of multiple factors beyond biology alone, including differences in cardiovascular disease burden, access to healthcare, socioeconomic conditions, and lifelong exposure to health inequities. Additional research is needed to better understand these disparities and identify effective interventions.

Building on Previous Dementia Research

The findings complement previous research showing that many dementia risk factors are potentially modifiable.

Recent international research has suggested that addressing multiple factors throughout life including hearing loss, obesity, depression, physical inactivity, excessive alcohol use, social isolation, and cardiovascular disease could prevent or delay a substantial proportion of dementia cases.

The current study narrows the focus to three common conditions that healthcare professionals routinely screen for and treat in primary care settings.

Dr. Jacqui Hanley, head of research funding at Alzheimer’s Research UK, who was not involved in the study, said the research offers a meaningful way to think about dementia prevention by focusing on the number of years people can expect to live without cognitive impairment rather than simply measuring disease risk.

She noted that while there is no guaranteed way to prevent dementia, growing evidence suggests that addressing cardiovascular risk factors including high blood pressure, diabetes, and smoking may help reduce the risk of cognitive decline.

What It Means for Clinical Practice

For healthcare professionals, the study underscores the value of preventive care during middle age.

Routine blood pressure screening, diabetes prevention and management, smoking cessation counseling, and cardiovascular risk assessment have long been cornerstones of primary care. This research suggests those conversations may also influence brain health decades later. The findings also support a broader shift toward discussing dementia prevention as part of comprehensive cardiovascular care rather than treating cognitive decline as an inevitable consequence of aging.

Importantly, the study does not suggest that controlling these risk factors eliminates dementia risk. Genetics, age, environmental exposures, and other medical conditions continue to play important roles. However, identifying modifiable factors provides clinicians with practical opportunities to help patients reduce their overall risk.

A Growing Public Health Priority

Dementia remains one of the fastest-growing public health challenges worldwide.  According to the Alzheimer’s Association an estimated 7.2 million Americans age 65 and older in the United States live with Alzheimer’s dementia today. This number could grow to 13.8 million by 2060, barring the development of medical breakthroughs to prevent or cure AD, underscoring the need for effective prevention strategies alongside advances in treatment.

Previous research has estimated that nearly 45% of dementia cases could potentially be prevented or delayed by addressing modifiable risk factors across the lifespan. The latest findings strengthen that evidence by identifying three common cardiovascular conditions that emerge during middle age and appear to have a lasting influence on cognitive health.

Looking Ahead

As the global burden of dementia continues to grow, researchers are increasingly focused on identifying opportunities to intervene long before symptoms develop. This study adds to a growing body of evidence suggesting that brain health and cardiovascular health are closely connected throughout adulthood.

While additional research is needed to determine how individual risk-factor modification influences long-term cognitive outcomes, the findings reinforce the importance of managing blood pressure, preventing type 2 diabetes, and avoiding tobacco use during midlife. These are interventions already recommended to reduce cardiovascular disease, and they may also help preserve cognitive function and independence later in life.

For healthcare professionals, the study serves as another reminder that conversations about cardiovascular risk extend beyond preventing heart attacks and strokes. The same preventive strategies discussed during routine primary care visits may also contribute to healthier brain aging for years to come.
